This patient has a tension nose deformity with a dorsal hump and an overprotected columella.

Procedure:
An external septorhinoplasty approach was used, a septoplasty performed and the caudal septum shaved to reduce columellar projection. The dorsal hump was excised, osteotomies performed and the nasal bones reduced. Lower lateral cartilages cephalically resected to refine the tip.
