This more mature patient has a dorsal hump and a very ptotic and bulbous nasal tip. The skin is quite sebaceous.

Procedure:
An external rhinoplasty approach was used. Hump reduced and osteotomies created. A septoplasty was performed and the upper lateral released and spreader grafts placed. A columellar strut graft was placed and the tip refashioned with cephalic resection and intradomal suture techniques.
